Why practices use TakeHelm

Patients follow through. Providers see the whole picture. Your practice grows.

One platform sits between your EHR and your patients’ daily lives, so the plan from each visit keeps working all week.

Patients

Understand their health, keep their information organized, and put their treatment plan into practice between visits, with guidance on nutrition and fitness, help working through barriers, and support for goals and accountability.

Providers

A provider dashboard with each patient’s full health picture: labs, wearables, nutrition, training and check-ins in one place. Ask our literature research engine questions about a patient’s chart and get answers backed by published research, with citations. Less administrative work, and visits that start further along.

Your practice

For your members · nutrition

Your care team sets the nutrition plan. Patients make it fit their life.

Your nutritionist or provider owns the targets and instructions. Each patient tells TakeHelm how they actually eat: their meals, routine, the foods they like and the time they have to cook. TakeHelm builds a nutrition plan with sample days that fits both, and tracks the week against your targets.

  • Works from a nutritionist’s targets and meal plans, or a provider’s recommendations.
  • Patients shape the plan around their own routine and tastes; your targets stay yours.
  • No nutritionist on staff? Patients still get a plan to follow.

For your members · fitness

An exercise plan built around each patient.

TakeHelm builds each patient’s exercise plan from their goals, the equipment they have and the time they can give it, and folds in your care team’s recommendations. If you have an exercise physiologist, their prescription and testing anchor the plan.

  • Built from the patient’s goals, equipment and schedule.
  • Incorporates your team’s recommendations and any exercise prescription.
  • No exercise program in-house? Patients still get a plan that fits.
  • Supported wearables add optional activity and recovery context.

For your members · every week

A weekly check-in that keeps them going.

Each week TakeHelm recognizes what worked, names one useful focus, and asks a question worth answering. The reply carries into next week.

  • Progress is measured against your plan, not a generic score.
  • Labs, genetics, wearables and body composition add context when available.
  • Questions get answered with the member’s whole history.

When it’s clinical

Hands off to your team cleanly.

TakeHelm knows when a question belongs to your clinicians, and offers the patient two routes: add it to the agenda for their next appointment, or send your team a portal message on their behalf, written with the relevant history and recent data.

  • The patient chooses the route. Nothing is sent without their OK.
  • Your team sees the context, not just the question.
  • No implied monitoring or response-time promises.

For your patients · before each visit

Visits that start with the right questions.

Patients keep a running agenda for their next appointment. Questions that come up between visits collect in one place instead of in scattered messages, ready for your team to review before the patient walks in.

  • Patients add topics themselves, or TakeHelm suggests one when a question needs your team.
  • Each topic notes who it’s for, such as primary care or nutrition.
  • Fewer between-visit messages, and fewer questions forgotten in the room.

For your practice

Built to make your membership work harder.

What TakeHelm is designed to do for the practice, not just the patient.

A membership members feel every week

Your program shows up between visits, not just at them. Built for engagement and renewal.

Fewer “what does this mean?” messages

Routine interpretation questions get answered with the patient’s full context. The ones that need you arrive summarized.

Visits that start further along

Patients arrive having followed the plan, with the data to show for it, instead of a blank slate.

A new between-visit program

Practices without a membership can launch a between-visit tier with TakeHelm inside it.

Your workflow

Your clinicians stay in charge.

We prepare each patient’s profile from your records before they ever sign in, and your team has its own view of every enrolled patient.

  • Works with Cerbo

Labs, plans and visit summaries flow from your records. No double entry.

  • You choose who enrolls

Roll out by provider, location or program, at your own pace.

  • A provider dashboard

Each patient’s full health picture in one place: labs, wearables, nutrition, training and check-ins, behind two-factor sign-in.

For your clinicians · research

The literature, read against this patient’s chart.

Ask a clinical question about a patient. TakeHelm searches the published research, reads the most relevant studies, and answers with that patient’s labs, history and plan in view. Every point links to the study behind it.

  • Grounded in published research

Searches millions of indexed papers through Europe PMC and PubMed, not the open web.

  • Specific to the patient

Findings are weighed against the patient’s own results, history and current plan.

  • Every claim cited

Each answer shows its sources, with the study type, citation count and access level.

  • Identifiers stay in the chart

Searches are built without patient identifiers before anything leaves TakeHelm.

TakeHelm for individuals

Good to know

Questions practices ask.

What does it cost?
TakeHelm is priced per enrolled patient per month, with a one-time implementation fee. Most practices include it in membership; some offer it as its own tier. We’ll walk through pricing on the call.
Do we need to change EHRs?
No. TakeHelm works with Cerbo today: labs, plans and visit summaries come from your records, with no double entry. On a different EHR? Tell us on the walkthrough call and we’ll go over upcoming EHR support.
Does it replace our patient portal or app?
No. TakeHelm works alongside your portal. When a patient chooses to send your team a question, TakeHelm writes the message with the relevant context and delivers it through the patient portal you already use.
Will TakeHelm market to our patients?
No. Patients who join through your practice are your patients. TakeHelm doesn’t send them offers for its individual subscription.
Who answers clinical questions?
Your clinicians. When a question needs clinical judgment, TakeHelm says so and offers two routes: add it to the agenda for the next appointment, or send your team a portal message on the patient’s behalf. The patient chooses, and nothing is sent without their OK.
How is this different from TakeHelm for individuals?

TakeHelm for clinics automatically brings in each patient’s health record, labs and visit summaries from your EHR, then layers on their wearables and tracking.
